This document explains how we use the personal information we collect about you. To provide healthcare for you, we will need to hold personal data of both a general and specific healthcare nature. We take your privacy seriously and only use this information to provide our services. Within our organisation the data will be held by Mr John Ivory and his secretarial/support team.
Processing of your data is necessary for the direct healthcare that we provide for you. There is a legal basis for this which is summarised as follows:
Necessary in the exercise of official authority invested in the data controller (Article 6 (1)c)
To comply with a legal obligation to which the data controller is subject (Article 6 (1)e)
As a special condition for direct healthcare, necessary for the purposes of diagnosis, provision of healthcare and treatment (Article 9(2)h.
We adopt a transparent approach to the processing of your personal data. We may need to share your data with other organisations such as your GP and hospitals for direct healthcare purposes. This might be by letter or electronically.
You have a general right to object to your data being processed in certain circumstances, such as if the retention of data was not for the safe provision of healthcare.
You have the right to complain to the Information Commissioner’s Office (ICO).

Data (medical records) will be retained for a minimum of 10 years but may be held beyond this so that information is available to process any potential clinical negligence claim.
